U.S. PUBLIC HEALTH SERVICE MALE DIRECTOR OR SENIOR GRADE OFFCER COMBINATION CAP

The visor ornamentation on the Combination Cap worn by both male and female members of the Commissioned Corps in the United States Public Health reflects the rank of the wearer. Corps personnel who have reached the rank of O-5 (Senior Grade) and O-6 (Director Grade) are authorized a single row of embroidered gold oak leaves and acorns on the visor, with lower-ranking Officers wearing unadorned visors and Flag Officers (Admirals) wearing two rows.

A gold chin strap with a 1/16-inch maroon band running its entire length is worn on this and all male Combination Caps, as well as on all female Alternate Combination Caps and on the old female Combination Caps (“bucket covers”) when worn by O-4 Officers and below.

A Combination Cap is a required component of the Blue and White versions of the Dinner Dress, Full Dress, and Service Dress Uniforms, as well as the Summer White. Its wear is optional with the Dinner Dress Blue Jacket/White Jacket Uniforms, and is permitted only when outer garments are worn.
