The Khaki Combination Cap for Director (O-5) and Senior Grade (O-6) Officers in the U.S. Public Health Service Commissioned Corps are distinguished from those worn by lower-ranked officers by the addition of a row acorn and oak leaves, crafted from gold bullion and embroidered on the cap’s visor. (Note: Equivalent ranks in the Coast Guard and Navy are O-5 Commander and O-6 Captain.)
