The 4th Evacuation Hospital Distinctive Unit Insignia was first approved on 8 July 1969 for the Army’s 4th Surgical Hospital. In March 1974 it was redesignated for the 4th Combat Support Hospital, and just over a decade was redesignated yet again for the 4th Evacuation Hospital.

Maroon and white are the insignia’s predominant colors and reflect its status as part of the Army Medical Department. Centered in the field is a large, red cross; always a symbol of humanitarian principles, it is used here also to convey steadfastness and caring. The interlacings form a series of “4s” in the cross’ center, reflecting the unit’s numerical designation, which is subtly emphasized with the four points of the cross. A disc containing the interlacing recalls a wheel and is an allusion to the critical mobile nature of the unit’ assignment.

The motto of “Beyond The Ordinary” is brought to life in the insignia by the inclusion of heraldic imagery evoking the numerous honors the unit has received over the years. Fleur-de-lis in the upper left and right corners stand for the 4th Evacuation Hospital’s participation in two World War I campaigns (St. Mihiel and Meuse-Argonne). Likewise,the four stars represent the unit’s credit for action in the Papua, New Guinea, Luzon, and Leyte campaigns. The Presidential Unit Citation the unit received for the Papua campaign and the Philippine Presidential Unit Citation it was awarded for the Philippine Island Campaigns (Luzon and Leyte) are represented by blue.
