The 137th Aviation Regiment Distinctive Unit Insignia—commonly known as a unit crest and referred to as a DUI for short—was approved on 1 December 1987, two months after the constitution of the 137th Aviation in the Ohio Army National Guard consisting solely of Company D. Its shield is crafted with ultramarine blue and golden orange, branch colors of Arm Aviation, along with white (silver) for the charge of a winged spear and embattled divisor. The embattlement suggests a fortress or castle wall and is an emblem of a strong defense, while the spear suggest the combat preparedness of the defense; its wings denote the ability to strike the enemy through aviation assets, and also suggest that the aviation mission can survive as the “spearhead” of an attack. The unit motto, "Maintaining Freedom," emphasizes the role that constant vigilance and readiness must play in order to preserve liberty against forces seeking its destruction.
Distinctive Unit Insignias are worn by all Soldiers (except General Officers) in units that have been authorized to be issued the device. It is worn centered on the shoulder loops of the Army Green Service Uniform (AGSU) and the blue Army Service Uniform (ASU, Enlisted only) with the base of the insignia toward the outside shoulder seam. Full guidance on wear of the DUI is found in DA Pamphlet 670-1, Section 21-22, "Distinctive unit insignia" and 21–3(d) and (e), "Beret" and "Garrison Cap," respectively.

Between 1990 and 2006, the 137th Aviation Regiment was reorganized seven times, changing the number of battalions and/or companies in the Regiment and the states in which the Regiment would be organized. It has been reorganized several times since then, but the particulars are not readily available and the unit’s Lineage and Honors page at the U.S. Center of Military History does not have data past 2010. As of Summer 2023, it is organized in the Ohio and Indiana Army National Guard and consists of the 1st Battalion, part of the 38th Combat Aviation Brigade, 38th Infantry Division. The Battalion's C Company is assigned to the Indiana Army National Guard, while its D and G Companies serve in the Ohio Army National Guard.
In addition to an Army Super Unit Award for the years 2004-2005, the 137th Aviation Regiment was also the recipient of a Meritorious Unit commendation for its 2009 deployment to Iraq, where its Aviation assets were used in direct support of the U.S. Ambassador’s Office and State Department, as well as commanding generals of Multi-National Force-Iraq, Multi-National Corps-Iraq, and Multi-National Security Transition Command-Iraq.
